You are not too busy. You are unevenly busy.

Two findings from the American Dental Association sit awkwardly together.

A third of owner dentists — 33.3% — said they were not busy enough and could treat more patients. Yet when asked to name their priorities, only 3.7% volunteered growing patient numbers.

That is not a contradiction so much as a symptom. Capacity problems feel like scheduling problems, so they get handled at the front desk rather than treated as a marketing question.

The gaps are not random

Most practices with spare capacity do not have a uniformly empty diary. They have specific holes: the 2pm on a Thursday, the week after a public holiday, the hygienist whose recalls stopped going out six months ago.

Those gaps have causes you can name. A recall list that nobody works. A cancellation with no waiting list behind it. A new patient enquiry that arrived at 7pm and was answered at 10am.

Each one is fixable. None of them is fixed by a brand campaign.

Most practices cannot see where patients come from

One UK survey of dentists found 40% do not track where new patients come from, and 30% do not know how many new patients they see in a month.

If that is you, every marketing decision after this one is a guess. You cannot tell whether the directory listing is working, whether the referrals are growing or shrinking, or whether last month was good because of something you did or because of the weather.

Tracking the source of every new patient costs nothing but a field on the intake form and the discipline to fill it in.

Referrals are still first, but they are slipping

In a survey of 448 practices, word of mouth and referrals were named the top new-patient source by 77.5% in 2025. This year it was 65.6%.

Google is taking up the slack. The same data put Google Business Profile as the top channel for 15.6% of practices, with Google Ads adding 7.6%.

The practical reading: referrals will keep arriving, but they are no longer enough on their own, and the place people check before booking is increasingly a profile you control.

Start with the boring half

Before any new campaign, work the list you already have. Recalls that have lapsed. Patients who cancelled and never rebooked. Treatment plans that were accepted and never scheduled.

It is unglamorous and it is almost always the cheapest chair you will fill this month.
